1.) I consent to and approve my child/children's taking part in any an all activities conducted by First Southern Baptist Church, Overgaard (FSBCO) AWANA Clubs. I understand that my child/children may participate in physical activities such as those held during Game Time.  As with any physical activity, there is a risk of injury.  I fully accept this risk and consent to the treatment of any minor injuries of my child/children, and release, hold harmless and indemnify AWANA and FSBCO and their officers, directors, agents, employess and volunteers from any and all liability, claims and costs arising from or growing out of such treatment.  In the event of an emergency that requires medical treatment for the named child/children above, I understand every effort will be made to contact me or my emergency contact.  However, if I/we cannot be reached, I give my permission to the AWANA volunteers to secure the service of a licensed physician to proved the care necessary for my child's well being.  I assume responsibility for all costs connected to any accident or treatment of my child.
2.) I understand that any off-property AWANA excursions will be communicated with me beforehand and I will be required to sign a seperate medical release form in order for my child to participate in those excursions.
3.) I give permission for photo(s) of my child/children to appear among other general club photos in any and all media as long as there is no identifying information published by FSBCO.  I hereby waive any causes of action I may have because of my child's/children's photograph.
4.) I acknowledge that FSBCO has many safeguards in place to protect the health and safety of participants regarding COVID-19 and other infectious diseases. However, health and safety cannot be guaranteed and participation is voluntary. 

The cost of the club is a flat fee of $10 per clubber, for the entire year.  There are $1 dues that kids get extra credit for on each club night.

CONFIDENTIAL FINANCIAL ASSISTANCE
No child will go without required handbook/uniform because of financial hardship.  I you are unable to pay some or all of the registration fees for the club, we will offer a delayed payment plan, or a possible full or partial scholarship.

There is a limited amount of scholarship funding available.  Please only request on if you really require assistance for your child to participate.  We still encourage your child to bring their weekly dues even if it is not the full $1.  The will help cover their awards and prizes that they can earn throughtout the year and other expenses the we incur to sponsor this AWANA Club.  They also earn points each week for bringing their weekly dues.


Fees can be collected once the club year begins.
